The staff at Wisconsin Surgery Center is dedicated to ensuring the best possible outcome for your procedure, and every procedure performed by our physicians. We believe that being prepared is the first step towards realizing that best outcome.
With that in mind, we ask that you familiarize yourself with the following information.
- Before your procedure, it's important that you carefully follow our instructions for preparing for surgery, as well as what you need to do and bring with you on the day of your procedure.
- Also, please be sure to share the information about what to expect after your surgery with those who will care for you after you leave our facility.

- Lastly, you'll find some guidelines on your rights, and responsibilities, as a patient of Wisconsin Surgery Center.

Advanced Directives
It is the policy of the Wisconsin Surgery Center to rescue every patient who should need resuscitation. After stabilization the patient will be transported to an appropriate acute care facility. However, any Advanced Directive given by the patient to the center is recorded in the patient's medical record and is reviewed periodically with the patient or his/her surrogate decision maker. It is the patient's responsibility to supply the center with their Advanced Directive for recording in their medical record.
*What are Advanced Directives,
An Advanced Directive is a written instruction that you make while you are mentally competent that states how you want your health care decisions to be made if you became incapacitated or cannot express your wishes. Advanced Directives guide your physician and other health care professionals, and relieve your family and friends from the burden of guessing what types of care and treatment you would want to receive.
