- To be treated with respect, consideration, and dignity and to be free from Abuse, neglect, harassment and discrimination.
- To privacy when receiving care, and to have your medical records held in confidence. You have the right to approve or refuse the release of these records.
- To select your own surgeon.
- To participate in decisions regarding your healthcare, and to receive complete information regarding your condition, prognosis, and treatment.
- To consult with other health professionals when you feel such a consultation is necessary.
- To consent to, or refuse involvement in medical studies or research projects conducted by Wisconsin Surgery Center.
- To be informed of business relationships between Wisconsin Surgery Center and other healthcare providers, insofar as your care is concerned.
- To be informed of policies and regulations by which you are expected to abide.
- To an Advanced Directive, including information on Advanced Directives and honoring Advanced Directives.
- To file a complaint/grievance, the grievance process and who to contact for a complaint, including a state agency.
- To file a complaint/grievance without fear of reprisal.
- To have a legal representative exercise the patient rights on the patients behalf.

- To supply accurate and complete medical history information.
- To inform Wisconsin Surgery Center staff if their instructions, information, or answers to questions are unclear or too difficult to understand.
- To fulfill any financial obligations you incur.
- To make follow-up appointments with your surgeon or medical specialist as directed.
